Requesting an Absentee Ballot

If you receive an absentee ballot request in the mail, I recommend not using these.  In most cases when I receive these they are incomplete and I can not send you an absentee ballot.  Instead please go to www.myvote.wi.gov and request and absentee ballot.  You may have to upload a copy of your license.  Or, please use the form below and mail or email that back to me with a copy of your ID.
